Established in 1980 as a 60-bed THQ hospital. It was later upgraded to a 125-bed DHQ hospital in 1993 and now has 250 beds (2017). The hospital is located at Kolo Tarar Road, Hafizabad. The total area of ??this hospital is 87 kanals. The hospital not only provides services to the residents of Hafizabad city but also serves as a referral hospital for the population of Hafizabad district and adjoining villages of over 1.1 million.

The District Headquarters Hospital is headed by a Medical Superintendent who is a Grade 18 officer. It is assisted by Additional Medical Superintendents and New Management Structure (NMS) officers. DHQ Hospital Hafizabad is a Punjab Healthcare Commission (PHC) certified health facility. The hospital has also obtained ISO-9001 certification from Direct Assessment Services (DAS).
